Project Description:

The Money Follows the Person will design a comprehensive roadmap with person-centered planning as the core framework. The goal is to develop a range of culturally appropriate Home Community Based Service that are sustainable for eligible individuals to transition and receive quality care in a community of their choice. The objective is to: 1) Promote eligible individual to choose where they receive healthcare in a community of their choice; 2) Identify and build capacity in Home Community Based Services (HCBS) that will support transitional services; 3) Ensure protocols are in place to provide equitable and quality assurance for eligible individuals.
