Project Description:

Training clinical professionals in the delivery of effective health care strategies for persons with disabilities and chronic health care needs. The focus is assisting clinical professionals with effective strategies for assisting adolescent patients and family members transition from pediatric to adult healthcare. In addition, training is provided to address transition needs of adolescents with disabilities and special health care needs that include employment, education, postsecondary education, health maintenance/management and other quality of life issues.
