Wyoming Case Manager Training Program

Project Description:
Case managers in the state of Wyoming need additional training and certification to serve individuals more appropriately with disabilities. The University of Wyoming – Wyoming Institute for Disabilities (WIND) proposes to develop and deliver a certification curriculum and process to meet this need. We will utilize best and evidence-based practices, participants' personal experience, and robust evaluation to develop and deliver a rigorous training and capacity-building program. Case managers who participate in the training program will increase their individual knowledge and skills to provide quality and person-centered support that results in full community inclusion, community membership, independence, productivity, and social participation for individuals and their families. At a system level, this project will lead to increased capacity of Case Managers to assist individuals and families in securing and supporting person-centered services, more person-centered support, increased capacity of case providers to manage challenges in a person-centered manner and reduced involvement of the state as a mediator. In consultation with the Home and Community Based Services (HCBS) Section of the Wyoming Department of Health, the WIND project team may prioritize and adjust activities and deliverable dates in order to accomplish the overall project goal. Adjustments to the priority and timing of activities listed below reflect the dynamic nature of the project.
