Project Description:

Shared Support, Inc. is a leading provider of services to people living with intellectual and developmental disabilities. Shared Support, Inc. is licensed by the Department of Human Services and partners with various counties throughout Pennsylvania to deliver high quality, innovative services specifically designed to fit the unique needs of each person we support. The Charting the LifeCourse (CtLC) framework is a set of principles and related tools that supports people and families to explore possibilities, identify their vision for a good life, brainstorm and decide what they may need to reach their vision, and connect with integrated supports and services to help in that journey. The LifeCourse Nexus is a National Training and Technical Assistance Center connecting and equipping people and organizations to use the Charting the LifeCourse framework for planning, problem-solving, and decision making. Becoming an "affiliate" of the LifeCourse Nexus provides organizations with strategic process consulting to establish a standard of practice/programmatic implementation plans for integrating the CtLC.
