Project Description:
Growing the Community Integrated Network of Oregon (CINO) to Deliver and Sustain Evidence Based Chronic Disease Self-Management Education and Support Programs. This is a subcontract on an ACL-funded grant to Comagine Health. Comagine Health is partnering with the Oregon Wellness Network (OWN), Oregon State University Extension Service (OSU Extension), Oregon Health & Science University's Oregon Office on Disability and Health (OODH), Oregon Health Authority (OHA), Connect Oregon CIE, Welld Health, Familias en Acción, Freedmen's Health to establish a statewide, coordinated, and sustainable Community Integrated Network of Oregon (CINO) as part of the Administration on Community Living Growing the Community Integrated Network of Oregon to Deliver and Sustain Evidence Based Chronic Disease Self-Management Education and Support Programs Grant. In addition to building infrastructure capacity, Comagine Health and Oregon Wellness Network will work with partners to scale capacity over the three-year period for the Self-Management Resource Center (SMRC) Chronic Disease Self-Management Program (CDSMP), Diabetes Self-Management Program (DSMP), Tomando Control de su Salud, Programa de Manejo Personal de la Diabetes, and Walk with Ease (WWE) Group and Self-Directed Enhanced. As part of this effort, the partnership is committed to increasing access and inclusion of populations disproportionately impacted by chronic disease, including adults living with disabilities. The Oregon Office on Disability and Health will work with key partners in providing training and technical assistance to ensure that program/intervention facilitators and program staff offer inclusive and accessible evidence-based health promotion interventions for older adults and people with disabilities.
