The Pine Belt Transition Together Enhancing young Adult services in Mississippis (TEAM) goal is to establish a plan to create a network of resources to increase community-based outcomes for youth and young adults with intellectual and other developmental disabilities (ID/DD) during the transition to adulthood. To accomplish this goal, the Pine Belt Transition TEAM will continue partnerships of the formed inclusive advisory team to develop a transition plan. This advisory team consists of self-advocates (youth and young adults), family members and professionals from community, government, nonprofit and transition-focused agencies/organizations. The Pine Belt Transition TEAM will focus on achieving three primary objectives. Objective One (1) will be the completion of an environmental scan to identify and gather information regarding current transition to adulthood services and resources in the Longleaf Pine Belt counties of Covington, Forrest, Greene, Jasper, Jefferson Davis, Jones, Lamar, Marion, Perry and Wayne. Objective Two (2) will be the development, dissemination and evaluation of a transition needs assessment to identify priorities of transition-aged youth and young adults with disabilities and their families. Objective Three (3) will be the coordination of a focus group to identity transition messaging focus areas for community awareness materials. A final evaluation will consist of a team review of all objectives and activities to develop a transition plan with a final outcome of establishing a high-quality, sustainable, community-based transition program.
