GLOVER: MOMS: MOMS Empaths

Project Description:

Empaths is a healthcare system-level approach in which all OB patients complete the 5 P's screener at three time points—their first OB visit, a visit between 28 and 32 weeks, and their first postpartum visit. Healthcare providers engaged patients who answer yes to at least one question in further conversation and refer to a centralized care manager, who provides further assessment, brief intervention, and referral to treatment via live or virtual visits. Patients get connected to appropriate behavioral health treatment services as well as peer and community support programming to promote healthy outcomes for moms and babies.
