Project Description:
The National Project staff will work in support of Community of Practice states to increase and enhance policies and practices that support family networks, enhance person centered planning in the context of the family, enhance support coordination, increase quality outcomes, expand service structures, and funding methodologies, and strengthen the role of families in all models of services.
NASDDDS and the UMKC Project Team will provide state-specific Technical Assistance and Consultation to develop and continue the understanding and implementation of Supporting Families Across the Life Span, based on the Charting the LifeCourse Framework and build capacity to apply it and systems level changes.
To achieve a more person-centered system, the Division of Services for People with Disabilities (DSPD) has been working with NCAPPS since 2019 on increasing stakeholder engagement, enhancing Person-Centered Support Planning standards and procedures, and exploring how to measure person-centered outcomes. DSPD plans to build on this work and focus on ensuring that person-centered practices are embedded across systems and the lifespan by pursuing the following goals:
Goal 1: By September 30, 2022, DSPD will enhance person-centered practices in all areas of the system through continued stakeholder engagement and revision of planning standards and procedures to ensure that person-centered thinking and planning are translated into practice.
Goal 2: By September 30, 2022, DSPD and the Utah Brain Injury Council (UBIC) will implement a train-the-trainer model to promote person-centered practices in services for people with a brain injury in Utah.
Goal 3: By September 30, 2022, youth with disabilities will be meaningfully engaged in person-centered planning and their various service plans will be aligned through collaboration and coordination between DSPD, the Utah State Board of Education (USBOE), the Utah Parent Center (UPC), the Utah State University (USU) Institute for Disability Research, Policy & Practice, and Vocational Rehabilitation (VR).
Goal 4: By September 30, 2022, the New Choices Waiver (NCW) team will revise and strengthen person-centered practices, policies, and procedures for older adults being supported through the Home and Community-Based Services (HCBS) NCW through increased stakeholder engagement, improved quality standards, and additional training for case managers and providers.
