Project Description:
The My Health. My Wellness. Teaching Guide project has two goals. First, the project will focus on developing and piloting the My Health. My Wellness. (MHMW) Teaching Guide through community pilot events in order to collect stakeholder feedback and quality-improve the model. Second, the project will use these community events and teaching guide to disseminate existing MHMW resources and tools among Delawareans with disabilities. This work will be conducted in three phases: (1) Phase I – MHMW Teaching Guide Development; (2) Phase II – MHMW Teaching Guide Dissemination; and (3) Phase III – MHMW Teaching Guide Quality Improvement.
