Project Description:
The goal of this proposal is to advance training opportunities for individuals, families, professionals and care providers on issues related to designing effective treatment regimens for individuals with disabilities transition of care from a pediatric to adult health care system. The intent is to expand existing successful training courses, so they may incorporate home and community settings that are separate from the medical care site but highly influential in the way a treatment is implemented and deemed effective. To meet this goal, we will create a new video and simulation segment that is family-led to be used in multiple training avenues. The video and complementary training will focus on the transitions of medical care from pediatric to adult providers, training future pediatricians and adult clinicians in transition care. The video will highlight the experiences of individuals, families and physicians who have transitioned from pediatric to adult providers. It will also provide individuals and families valuable information to prepare themselves and their care teams. The project will also work to implement a family-led ECHO training series emphasizing medical transitions that will facilitate discussions among students, current providers, and families. The ECHO series will support discussions about obstacles and strategies to address transitioning from pediatric to adult health care providers. The focus of this series will be "Navigating Medical Transitions into Adulthood".
