Project Description:

The activities funded by this award will improve access to COVID-19 vaccines to include the following activities: Education about the importance of receiving a vaccine; Identifying people unable to independently travel to a site; Coordinating a vaccine appointment; Providing mobile vaccine clinics for clients who are unable or unwilling to travel or do not have access to accessible transportation; Reminding people of their second vaccination appointment if needed;
