Project Description:
Over the past decade, research has accrued information about healthy communities - or accountable healthy communities research. This research has shown that environments that address social services (transportation, housing, utilities, interpersonal violence, food insecurity) among patient populations improves patient compliance and adherence to medical regimen and, in turn, improves health outcomes, particularly for underserved settings. This study is a randomized control trial funded by CMS and with partnership with the Partners in Health medical association in WV. WVU CED provides the patient navigation and services to address social services for this study.
