The proposed project will enhance the OODH Linkage Demonstration Project in several ways that are critical to project success. As a demonstration project, this work will begin regionally as a model is developed and strategies evaluated. To meet state needs known to be concentrated outside of the Portland metropolitan area, the Linkage Demonstration Project Enhancement will concentrate efforts in two geographic regions, southwest Oregon in Year 1 (Curry, Coos, Josephine, Jackson, and Douglas Counties), and expanding to the mid-western valley and coast in Year 2 (Lane, Linn, Benton, Lincoln, Polk, Marion, Yamhill, and Tillamook Counties). Enhancements above and beyond the activities funded by CDC include: (1) A survey of case managers from case management entities and brokerage agencies in Oregon to ensure that the program supports rather than duplicates the work of case management. (2) A training module specific to mental health care providers will be developed, and six quarterly mental health provider trainings will be conducted by the OODH Health Educator and Community Co-Trainers with I/DD. (3) Peer mentors recruited from the geographic regions of Oregon to be address by the proposed project will be trained to provide support for linkage participants that is informed by lived experience. (4) Linkage staff will develop partnerships in the geographic region to create resources and address barriers identified in the case manager survey
