Project Description:

CHD will assist the lead agency, Governors Council on Special Education and Disabilities, and other partners with the following activities: review, analysis and report of HCBS Plans of Care policies, guidance, and training; review, analysis and report of Plans of Care emergency plans; evaluation of pilot projects in two DD agencies; obtain baseline of incarcerated individuals with IDD; develop and implement training module and toolkit for DSPs; support implementation of cultural and linguistic competence assessment; obtain baseline on supported decision making agreements and on Adult Protective Services Reports of Harm; develop, implement and analyze shared vision survey.
