Project Description:
The Healthy Heart Ambassador (HHA) supports participants with self-monitoring and tracking their blood pressure. Participating HHA Program will identify an ambassador who will: 1. Participate in HHA Training (required for all new Ambassadors). 2. Ensure HHA Program is listed prominently on the YMCA website. 3. Recruit new participants from: • CDQii healthcare system referrals • Community screening and education events conducted by the HHA. Organizations must conduct at least two community-based screening/education events. 4. Enroll participants which includes making sure the participant qualifies, completing forms and collecting a baseline (initial) reading 5. Take and record participant's blood pressure weekly. 6. Model and discuss proper techniques for monitoring blood pressure in person and virtually. 7. Encourage self-measured blood pressure, provide blood AHA My Blood Pressure Log, or recommend blood pressure monitoring application (such as the AHA BP Tracker app), and review entries weekly with participants. 8. Send weekly support messages to an assigned group of participants. 9. Present monthly Nutrition Seminar to participants. 10. Attend and participate in the MSDH HDSPP's Kick-off meeting, Bi-monthly meetings, Operational Site Visits, and three (3) Learning Sessions. 11. Collect health information in secure MSDH-HDSPP database and maintain confidentiality 12. Ask and answer questions from program participants and members to support them in achieving their goals related to health and well-being. 13. Maintain working knowledge of blood pressure and trends to provide effective information and support to members. 14. Build effective, authentic relationships with members; helps members and program participants connect with each other, their primary care providers, and the wellness program. 15. Observe and adjust approach to support all participants' capabilities, physical conditions, health, and culture. 16. Celebrate achievement of program participants related to program or personal goals, mastering of specific skills or overall health and well-being. 17. Ensure designated secure location and maintain accurate program records. 18. Submit invoices, along with reports and other supporting documentation, to the Mississippi State Department of Health, Heart Disease and Stroke Prevention Program on the 15th of each month.
