Project Description:
Collaborating with Family Medicine to educate providers and residents on the support needs of those transitioning to adult healthcare. We have done a QI survey to gauge the current confidence level of providers and residents and will follow up after 3-1-hour learning sessions. This training will provide young adults transitioning from pediatricians find adult providers while staying in the KU HealthSystem.
