Project Description:
Need: Home and Community-Based Services (HCBS) are person-centered services and supports delivered in one's home and community. The Centers for Medicare and Medicaid Services HCBS Rule ensures individuals with disabilities live in integrated communities, work in competitive integrated settings, engage in community life, control personal resources, and receive services in the community. This project will prepare individuals with disabilities, their families, and providers to understand the values of the HCBS Rule when receiving HCBS supports with a specific focus on individuals moving from family homes and licensed homes into homes of their own. Goal: To prepare individuals with disabilities, their families, and providers to understand the values of the HCBS Rule when receiving HCBS supports with a specific focus on individuals moving from family homes and licensed homes into homes of their own. Objectives: 1. Develop training to increase awareness of individuals with homes of their own. Objectives: 1. Develop training to increase awareness of individuals with disabilities, family members, and service providers on community living for people with disabilities. 2. Deliver training to increase awareness of individuals with disabilities, family members, and services providers on integrated, affordable, and accessible living arrangements for people with disabilities. 3. Community Trainers will mentor individuals with disabilities to transition from family homes and licensed homes into homes of their own. 4: Evaluate the project's outcomes and results. Expected benefits a) Increase awareness of individuals with disabilities and family members about living in one's home and community. b) Individuals have more control of their lives (how they spend their day, where they live). c) Individuals with disabilities are mentored to transition from family homes and congregate living arrangements into a home of their own. d) Increase understanding of community providers to support individuals to live in a home of their own.
