Project Description:
American Samoa does not have Home Community Base Services (HCBS). The proposal aims to establish a system for eligible individuals receiving Long-Term Support Services (LTSS) with the option to transition back into the community rather than remain in an institution. LTSS is qualified individuals who stay in an institution such as LBJ Tropical Medical Center or Hope Hose for at least 60 days.
The Money Follows the Person (MFP) project will develop a GAP Analysis and a comprehensive road map during years 1 and 2. During the planning phase, the HCBS committee will establish and design questionnaires to collect data in a brainstorming session. The analysis will determine the project needs and capacity-building activities for years two to three. Activities may include but are not limited to support home health care: nursing care, therapies in physical, speech, occupational, and dietary management.
The implementation phase will take place in years 3-5.
The UCEDD, the recipient of the grant, does not provide the services. However, it will connect the individual needs to the services in the community, such as Independent Living, Home health care services, etc., to transition smoothly. What does not exist to support HCBS will be identified and added to the list of capacity building.
The MFP can cover temporary food security, housing assistance, and home modification if necessary. The support cannot exceed 365 days.
The objective of the grant is:
1. Establish a Long-Term Support Services Advisory to design a comprehensive road-map to community living
2. Conduct a comprehensive system assessment and GAP Analysis to identify capacity-building needs;
3. Develop resource materials such as person-centered planning, transition handbook, operational protocol, and PSA's.
