NH Wraparound Practice Model

The goal of this work is to provide training and coaching to NH Wraparound Coordinators who are providing the NH Wraparound model. The IOD will be responsible for training and coaching all new Wraparound Coordinators and agency staff who support the Wraparound Model for both the Families and Systems Together (FAST) Forward program, the Monadnock System of Care project, the Manchester System of Care project, as well as other organizations that are approved by the state agency to provide Wraparound. The IOD will work with project coordinators at the NH Department of Human Services and other stakeholders to design and schedule training sessions and provide individualized a group coaching. The IOD staff involved in this process were instrumental in designing and have experience delivering the NH Fast Forward Wraparound curriculum and is therefore well positioned to deliver these services.
