South Carolina Community Living Partnership Planning Grant
Project Description:
The South Carolina Community Living Partnership (SC CLP) embraces shared goals of enhancing community living, inclusion, and equity for South Carolinians with intellectual and developmental disabilities (ID/DD). The SC CLP is a coalition led by the UCEDD at the University of South Carolina with help from Able South Carolina, a Center for Independent Living. The SC CLP includes the SC Developmental Disabilities Network and other partners, including self-advocates, family members, educators, medical providers, service providers, and legal advocates.
The SC CLPs goal is to develop a three-year initial implementation plan to address system gaps and strengthen systems that promote independent living opportunities for youth and adults with ID/DD throughout South Carolina. The Project will have a two-pronged focus: (1) Preparing youth and adults with ID/DD for independent living through direct services and provider capacity building; (2) Exploring systems change opportunities to meet current needs and close gaps.
The SC CLP has four objectives to meet the goal within a 12-month timeframe. SC CLP will 1) Convene a state-level Partnership Team involving key stakeholders from existing coalitions and frameworks; 2) Conduct a needs assessment and environmental scan to determine gaps in transition to inclusive and integrated independent living; (3) Examine strategies and best practices related to transition in other states and successful pilots in South Carolina; (4) Develop recommendations that have the buy-in of state agencies and stakeholders. The anticipated outcome of the project would be a new, replicable model of how to build consensus around collaborative systems change for community living initiatives that includes identified curricula, planning guides, and data tracking tools.
