Project Description:
a. Recruit and maintain a network of at least 12 family members (min of 2 per DARS region) who have received Charting the LifeCourse (CtLC) Ambassador training from the University of Missouri-Kansas City as well as training on Pre-Employment Transition Services (Pre-ETS) from Virginia DARS. b. Work with DARS to develop the CFI Pre-ETS Family Ambassador program that provides peer supports to family members of Students with Disabilities (SWDs) participating in Pre-ETS that will foster student self-advocacy skills through scripted goal setting. Program development shall include: i. Identifying and tracking outcome measures; and ii. Developing and implementing a recruitment and matching process for individuals interested in the service (to include marketing and promotional materials); and iii. Developing a referral process for matching interested families with LifeCourse Family Ambassadors; and iv. Developing program forms and reports to be used for service delivery reporting, including referral forms, goal setting forms, invoices and reports. c. Disseminate information about the CFI Pre-ETS Family Ambassador program to transition stakeholders to promote awareness of the program and stimulate interest and referrals. i. Make a minimum of XX amount of contacts per quarter, XX amount of engagement activities per quarter, XX amount of trainings per quarter. ii. Maintain and submit accurate reporting of activities to DARS along with invoicing. d. Meet with DARS staff on an ongoing basis as needed to discuss the progress of the program
