Project Description:
This is a 3-year project that will sustain a collaboration of organizations involved in rural dementia care with an intended goal to increase capacity and improve access to person-centered, evidence-based support that helps caregivers and persons with ADRD and those with IDD and ADRD maximize their ability to effectively manage cognitive changes and extend their ability to maintain in the community setting. Led by OCCK, Inc., partners in this grant will strengthen dementia capacity by adapting the Care Ecosystem model to rural extension sites.
