Project Description:
Professionals supported through this program may provide a wide range of public health services and supports, including provision of culturally affirmative and linguistically accessible information, access assistance for vaccines and boosters, transition and diversion from high-risk congregate settings to community living, provision and connections to health and wellness programs, activities that address social isolation and social determinants of health, and other activities that support the public health and wellbeing of people with disabilities. Aligned with the overall purpose of the program, the Wyoming Institute for Disabilities proposes to: • Provide training to the 23 Public Health Nursing offices in Wyoming to work with individuals with disabilities and families • Include public health concepts into Friendship and Dating training. (3-4 courses) • Collaborate with the Wyoming Department of Health, Immunization Community Outreach Coordinator, to include the provision of culturally affirmative and linguistically accessible information to access assistance for vaccines and boosters, transition, and diversion from highrisk congregate settings to community living resources. • Develop public health course and competencies into direct support professional training. Content, leveraged from the Association of University Centers on Disability, the Center for Disease Control, and Rocky Mountain Public Health Training Institute, will be developed by the UCEDD and disseminated to agencies across the state. • Disseminated related information through the Wyoming Public Health Association. • Integrate core concepts of public health into current developmental disability waiver case manager training initiatives with home and community-based services.
