Project Description:
Milestone Five (5): Revise the Hourly Nursing Assessment Tool (H-NAT).
6.1.6.1 Northern Arizona University (NAU) agrees to revise the H-NAT and training materials to
accommodate an updated skilled nursing matrix.
• Participate in meeting to discuss revisions to the skilled nursing matrix,
• Revise H-NAT training materials to align with updated skilled nursing matrix,
• Provide guidance to the Division of Developmental Disabilities/Information Technology
Services (DDD/ITS) department on revisions to the H-NAT within the DDD system.
6.1.6.2 NAU agrees to continue to provide technical assistance on the H-NAT tool and inter-rater
Reliability (IRR) system.
• Provide ongoing communication with nurse managers and nurses for the
purpose of promoting consistent and reliable use of the H-NAT,
• Participate in bi-weekly team meetings with project team.
6.1.6.3 NAU agrees to continue to provide H-NAT training for the nurse managers and nurses.
• Develop an H-NAT training manual,
• Health Care Service Administrator will collaborate with nurse managers to determine
additional training needs and develop a plan for addressing the needs.
6.1.6.4 NAU agrees to continue to provide support on the H-NAT IRR system.
• Develop an auditing process for the IRR system that includes:
a. A clear description of nurse manager/nurse roles,
b. A system for ensuring new nurses/nurse managers are trained on the use of the
H-NAT,
c. Training sessions on how to interpret H-NAT IRR data,
d. Meetings with nurse managers to discuss solutions for addressing drift in inter-
Rater reliability.
6.1.7 Provide on-going technical assistance to DDD as needed.
10.1.5.1 Inter-rater reliability System:
• Quarterly reports for DDD Executive Leadership team and the DDD Nursing Program
Manager, and
• DDD and NAU will collaborate to determine what elements of reporting are needed.
10.1.5.2 A revised H-NAT with the new skilled nursing matrix:
• The H-NAT will be housed in a DDD system, it will list skilled nursing items approved by
DDD, and allow nurses to enter in appropriate information to determine an estimate of
Skilled nursing care,
• Modification of H-NAT components including detailed instructions for how to use the H-NAT
And a summary of how the H-NAT works in conjunction with other important documents
(e.g., Nursing Assessment) and DDD policies.
10.1.5.3 Revised H-NAT training materials:
• A training manual on the H-NAT for nurse managers,
• Train the trainer assessment materials, and
• Train the trainer documentation of protocols and structures of the train the trainer
approach.
