Regional Transition Navigator Program
Project Description:
The purpose of this program is to provide a team of professionals and activities designed to connect youth and young adults (14-25 years) with serious emotional disturbance (SED), serious mental illness (SMI), or Substance Use Disorder (SUD) with needed services and supports to develop independent living skills, create and cultivate natural supports, navigate the various systems within the state, access and participate in treatment and recovery services, and thrive in all eight dimensions of wellness including: emotional, spiritual, intellectual, physical, environmental, financial, occupational, and social.
