UA-Transitioning adolescents with special healthcare needs

Project Description:
Aim 1 will determine critical contributing factors that influence adolescent perceptions of and readiness for the transition process through questionnaires, medical record review and brief executive function assessment. Elements from the expanded SMART model and subsequent interviews will be used to measure factors previously proposed to contribute towards successful transition. Aim 2 will assess adolescent perceptions of the meaning and ownership of the transition process through focus groups of individuals in different age groups and disability categories. We will use fuzzy cognitive mapping to quantify and represent the transition process and weight critical elements in an effort to develop pathways for potential intervention to improve adolescent readiness and ability to adopt adult roles.
