Family Voices, SPAN, Got Transition and UMKC-IHD are partnering to develop a National Center for Empowering Youth with Intellectual and Developmental Disabilities to Manage Their Healthcare Transitions (HCT) on this five-year grant. This funding is intended to create and maintain a national person-centered, culturally competent resource center to empower youth with ID/DD in directing their own healthcare transitions from pediatric to adult models of care. Specifically, UMKC-IHD will partner with the project team to build out LifeCourse Online App for HCT, provide technical assistance on implementing the Community of Practice and will develop resource materials, utilizing the LifeCourse framework, on HCT.
