CHLA UCEDD Community-Based Transition Planning Pilot
Project Description:
The goal of this planning proposal is to develop an equitable, comprehensive plan for a community-based transition pilot program (CBTPP) for youth and young adults (YYA) with ID/DD in an underserved and racially and ethnically diverse Los Angeles community that addresses both transfer of medical care as well as transition into community-based services and supports to promote independent living and employment The CBTPP is envisioned to become a community-based outreach program of the USC UCEDD at CHLA. The objectives enacted according to the three phases of this project are: Phase 1: a) Convene the Partnership Advisory Team (PAT); b) Use formative and consensus-based process to foster PAT communication; c) Establish communication and resource infrastructure; d) Create four workgroups focused on creating CBTPP. Phase 2: a) Gather relevant state-level transition documents; b) Conduct comprehensive review and analysis of transition literature; c) Compile listing of transition resources; Phase 3: a) Develop and implement a CBTPP; b) Promote network of CB organizations and agencies; c) Create the linkages among transition and adult systems of care; and d) Develop plan of evaluation. Anticipated Outcomes include: a) Development of CBTPP for implementation; b) Empowered racially and ethnically diverse YYA with ID/DD; c) Increased awareness of the transition needs of YYA with ID/DD; d) CBTPP blueprint for replication; e) Interagency collaboration fostered; and f) Compilation of transition resources for CBTPP. Products include: a) CBTPP ready for implementation; b) Dissemination of transition resources to CBTPP and USC UCEDD network; c) Transition Summit; and d) 2 white papers on transition process and Transition Summit
