Project Description:
Need: Person Centered Practices (PCP) has emerged as a foundation for people with Intellectual and Developmental Disabilities (IDD) to create plans based on how the person chooses to live their life. People who receive HCBS waiver services are required to develop a person-centered plan, outline their life outcomes and how the person will actualize those outcomes. Overall goals and objectives: The Center will provide person centered training and technical assistance for a wide range of organizations to embed PCP practices within the organization. The Center will provide training on PCP specific to the required elements of a person-centered plan including: Person Centered Thinking, Plan Facilitation and People Planning Together. These training efforts will support professionals and para-professionals to provide supports using PCP. Unusual features: The Center is actively providing training for people with IDD to develop their own plans using a Peer to Peer model. Peer trainers facilitate the training and provide support to participants to develop person centered plans. These plans are introduced by the person with lived experience as a part of the person-centered planning process. Expected benefits: Professionals, family members and people receiving services will have an understanding regarding the PCP process, what their role is in the process and welcome peer to peer supports.
