Project Description:
the Partnership will convene a diverse group of youth with IDD, families, and cross-system professionals who will work collaboratively to 1) define domains of well-being for transition-aged youth with IDD including physical, mental, and social-emotional health, 2) identify indicators and measures for these domains; 3) examine available data and/or collect new data in areas such as racial disparities in transition outcomes and the impact of COVID-19 on transition, to identify the most pressing needs/concerns related to well-being for transition-aged youth with IDD, and 4) develop a project implementation plan to enhance selected well-being domains through a collaborative project with a community-based organization (CBO). The long-term goal of this transition planning project is to increase well-being among transition-aged youth with IDD and the primary outcome for the one-year project is a comprehensive implementation plan focused on addressing well-being for youth and young adults with IDD aged 18-24.
