Project Description:
The Administration for Community Living (ACL) with funding and partnership support from the Center for Disease Control (CDC) provided one-time funding to improve vaccination access for Older adults and people with disabilities in the United States in response to Coronavirus Disease 2019 (COVID-19), as outlined in the Coronavirus Response and Relief Supplemental Appropriations Act, 2021, P.L. 116-260 SCOPE OF WORK: Education about the importance of receiving a vaccine; Identifying people unable to independently travel to a site; Helping with scheduling a vaccine appointment; Arranging or providing accessible transportation; Providing companion/personal support; Reminding people of their second vaccination appointment if needed; and/or Providing technical assistance to local health departments or other entities on vaccine accessibility.
