Project Description:
This Modification is to:
A) Exercise Optional task 5.5 for transition support
B) Extending the end date of the period of performance from 07/31/2022 to 9/23/2022.
C) Add funding to CLIN 20 for optional task 5.5, a funded obligation of $101,479.00.
D) Change the total funded value of this order from $584,171.00 to $685,650.00, a net
increase of $101,479.00.
E) Change Contract Specialist from Tim Wind to John Snyder
F) Change Contract Officer from Tim Wind to Paula Johnson.
G)In consideration of the modification(s) agreed to herin as complete equitable
adjustments for the extension, the Contractor hereby releases the Government from any and
all liability under this contract for further equitable adjustments attributable to suchfacts or circumstances giving rise to the change
and extension.

All other terms and conditions remain unchanged
and in full force and effect
