Our program addresses key challenges to VA-Medicaid coordination through the following three steps: 1) Creates communication channels and builds enthusiasm and connections between VA and state Medicaid program staff We use a proven intervention called COVER 2 COVER to cross-training VA and Medicaid staff, building relationships, identify gaps and developing a model of coordinating care. 2) Exchange data to identify co-enrolled/eligible Veterans We have created a Data Use Agreement to identify Veterans enrolled in Medicaid that is applicable to other states. The process capitalizes on databases used in every state to identify health insurance enrollment, and uses the High-Needs, High-Risk Veteran registry to identify Veterans at highest risk of long-term institutionalization to target for step 3. 3) Coordinates care to address gaps and duplication in services - With new partnerships in place between the VA and Medicaid programs there is a stronger collective strength ensuring Veterans have the services they need to age in place. Collaborations tailored to local needs and resources may include expanding VA Home-Based Primary Care to dually enrolled Veterans in exchange for more HCBS provision from Medicaid programs, or VA and Medicaid programs partnering to move eligible Veterans out of the nursing home and back into the community.
