Project Description:
Person-Centered Counseling is integral to assuring that the needs and preferences of older adults and people with disabilities are being identified and met. Effective person-centered counseling guides discussions that help people, families, and interdisciplinary teams to problem-solve and develop effective support strategies using both systems based services along with resources available throughout the community. The Centers for Medicare and Medicaid Services (CMS), Home and Community Based Services (HCBS) Final Rule, sets forth regulatory requirements that maximize opportunities for participants to access the benefits of community living and receive services in the most integrated services. The Final Rule requires that service plans must be developed through a person-centered counseling process that addresses health and long-term services and support needs in a manner that reflects individual preferences and goals.
