Project Description:
The University of Arizona's Department of Family & Community Medicine, Department of Pediatrics, and the Children's Clinics for Rehabilitative Services (CCRS), have begun planning a collaborative "best practice" model for transition of care from pediatric care at CCRS to adult care at the Alvernon Family Medicine Center, the residency practice site of the DFCM. The UCEDD has laid the ground work for a best practice model that ensures effective, patient-centered primary care for young adults with developmental disabilities. The clinical model will expand on the concept of the "medical home" developed locally by the Southwest Institute for Families and Children with Special Needs. In a medical home, a clinician works in partnership with the family/patient to assure that all of the medical and non-medical needs of the patient are met. Through this partnership, the clinician can help the family/patient access and coordinate specialty care, educational services, out-of-home care, family support, and other public and private community services that are important to the overall health of the child/youth and family.Impact of the patient centered medical home for adults with I/DD on health and cost outcomes.
