Project Description:
The overarching goal of the CHEER initiative is to empower Kentuckians with cognitive and mobility limitations to make healthy lifestyle choices in order to reduce the prevalence and impact of hypertension and associated conditions. Through the delivery and systemic integration of appropriately adapted interactive training in nutrition, physical fitness, community programming, and educational opportunities, coupled with on-going technical assistance, collaborations with Public Health and other key health partnerships, and community education, this initiative has been carefully designed to lead to measurable health outcomes, promote long-term health for Kentuckians with cognitive and mobility limitations, and ensure lasting community partnerships.
