Minnesota DDA LifeCourse Nexus

The LifeCourse Nexus team proposes the following activities to support the Minnesota DSD establish a statewide platform that allows for networking, connection, and shared learning of organizations who support families of people with disabilities. The foundational activities described below provide a deep understanding of the Charting the LifeCourse principles that will allow for the development of a common language and vision to enhance collaboration and partnerships. Build the capacity of lead agencies and provider organizations to implement person centered practices. The foundational activities described below provide a deep understanding of the Charting the LifeCourse principles that will allow for the development of a common language and vision to enhance collaboration and partnerships within regions, further developing the regional cohort model. Proposed activities include providing professional development and strategic planning to support organizations to use the Charting the LifeCourse framework in their practices.
