Help Me Grow - Los Angeles (HMG-LA) is implementing the Help Me Grow national approach in Los Angeles county, with the goal of promoting cross-sector collaboration to implement and strengthen early screening and surveillance of developmental and behavioral delays for all young children, and connect children with or at risk for delays and their families to the appropriate intervention services and supports. HMG-LA is led by a partnership between First 5 LA and the LA County Dept of Public Health, along with key stakeholders in the Systems Synergy Council. A key component of HMG is the implementation of a Centralized Access Point that provides a telephone and web-based hub to link children and their families to early intervention services and supports, provides information, screening, and referrals, and education about child developmental milestones. This contract is to provide technical assistance to HMG-LA regarding the development and implementation of the Centralized Access Point.
