Expanding Disabilities Networks (UCEDD) Access to COVID 19 Vaccines
Project Description:
Activities in this area will focus on increasing access to vaccines to individuals with disabilities and older adults in New Hampshire. This may include but is not limited to:
Support of local Area Agencies and other service organizations to provide direct support services for vaccine and healthcare access;
Stipends for expenses incurred accessing the vaccine;
Identifying people unable to independently travel to a site;
Helping with scheduling a vaccine appointment;
Arranging or providing accessible transportation;
Providing companion/personal support;
Vaccine education and outreach
Activities in this area will focus on developing, sharing, and disseminating resources and information on vaccines. This may include but is not limited to:
Education to diverse populations about the importance of receiving a vaccine;
Individuals with disabilities, older adults, Direct Support Professionals, families members, diverse communities
Reminding people of their second vaccination appointment if needed;
Reviewing and disseminating current ACL, CDC, and UCEDD materials to multiple audiences
Development of customized education and media for target populations
Local and state level assistance
Activities in this area will focus on supporting and strengthening current infrastructure that ensure equitable access to healthcare, information, tools, and resources. This may include but is not limited to:
Supporting local and state healthcare navigators
Providing technical assistance to local health departments or other entities on vaccine accessibility
Support of DD Act Partner activities to support equity and access to vaccines
Investment in current state public health strategies to address healthcare disparities
Incentives for target population receive vaccination
