Project Description:
The WV No Wrong Door System plan was developed and finalized by a statewide, comprehensive Steering Committee, ADRN Advisory Council and other stakeholders around the State in 2015. The purpose of this plan is to strengthen access to long-term services andsupports (L TSS) within the state with the goal of transforming this system into a centralized system that has multiple access points, a fully integrated person-centered approach, coordination of care, and an assessment approach that will help standardize and tailor services for all eligible individuals. Several environmental factors have kept this plan on the shelf since 2015. As a result, it will be important to review and evaluate the plan in the current context. Once modified as needed, we will reinstate the implementation process. The overall goal of this proposal is to improve access to the long-term services and supports in WV. We argue that the No Wrong Door System plan established in 2015 will continue to accurately depict the state infrastructure needs for improving L TSS in WV but that this plan may need to be modernized to include new partners and a few processes.
