Project Description:
Overall Goal: To improve access to quality person-centered home and community based services that help persons living with or at high risk of developing Alzheimers disease and related dementias (ADRD) and their caregivers to live independently and safely in their communities. Outcomes: 1. To increase access to effective care and supportive services to persons living with moderate to severe impairment from ADRD and their caregivers. 2. To improve the quality and effectiveness of programs and services dedicated to individuals aging with intellectual and developmental disabilities (IDD) with ADRD or those at high risk of developing ADRD. 3. To increase access to behavioral symptom management training and expert consultation for family caregivers. Objectives: 1. Leadership & Partnerships: To develop a leadership infrastructure and expand partnerships that builds the organizational capacity of the region to enhance and expand access to person-centered home and community based services for persons with ADRD and their caregivers. 2. Evidence-Based Interventions: To adapt and implement evidence-based exercise and behavior management programs for persons with ADRD and their caregivers that increases access to effective care and services. 3. Scalability & Sustainability: To create training and technological infrastructure that supports the ongoing development and sustainability of the project within the Kansas City region and nationally. 4. Capacity Building: To provide training, technical assistance, and coaching that enhances the capacity of home and community based service organizations to effectively serve individuals with moderate to severe impairment from ADRD, those aging with IDD with ADRD, or those at risk of developing ADRD. 5. Planning, Assessment, & Evaluation: To conduct a baseline assessment of the dementia capability of the existing system and to establish a monitoring and evaluation system, including metrics and indicators, that assists partners to gauge progress and supports ongoing program improvements.
