Project Description:
The Institute on Disability/UCEDD in collaboration with key partners (NH Council on Developmental Disabilities, Community Support Network, Inc., ABLE NH, People First NH, Disability Rights Center NH, and NH Bureau of Developmental Services) will, over the next five years, develop and implement the NH Quality Framework, a comprehensive and coordinated approach that increases and improves the quality of home and community based services for individuals with developmental disabilities. Proposed goals and objectives: Goal 1: Partners will actively engage in the full implementation of the project. Activity: Recruit Quality Council as advisory committee. Goal 2: The NH Quality Framework will demonstrate a comprehensive system to improve services for individuals living in the community. Activity: Develop outcome focused framework based on EBP strategies. Goal 3: DSPs and other providers will demonstrate improved competencies to improve access to and quality of HCBS services. Activities: Develop self-advocate led training and assess/improve organization based training. Goal 4: The NH monitoring system will demonstrate an increase in tools, training and other strategies to address abuse, neglect, and exploitation. Activities: Use EBP tool to assess system and develop effective rights training for individuals. Goal 5: DSPs and other providers will demonstrate increased understanding and follow through on health issues. Activities: Evaluate and improve use of HRST. Goal 6: The project evaluation will measure and report on activity progress, barriers, and areas for improvement. Activity: Conduct evaluation. Goal 7: The NH Quality Framework will demonstrate a sustainability plan that reflects organizational, financial, and community sustainability. Activities: Conduct sustainability planning throughout project. Develop final sustainability plan with MOU.
