Project Description:
IDD Transition Task Force. The UCEDD developed the Lifespan Transition Clinic (LTC) in partnership with the Oregon Center for Children and Youth with Special Health Care needs. The primary goal of this clinic is to help families 1) identify transition readiness; 2) prioritize transition goals; 3) identify mental health needs; and, 4) facilitate the steps to successfully transition to an adult system of care. Often this includes helping youth and families learn to use technology such as MyChart, learn to advocate for their needs, and make a plan to live well in the community.
