Project Description:
The purpose of the grant is to build the capacity of community-based organizations (CBOs) serving people with physical and/or intellectual or developmental disabilities to contract with integrated care and other health sector entities. With a team of national partners, the NASUAD and ACL aim to 1.Develop the baseline knowledge of current disability CBO involvement and ongoing needs to engage with the development and implementation of integrated care systems; 2.Provide broad-based training and technical assistance for disability CBOs that increases their capacity for operating effectively in integrated care systems; 3.Convene and provide targeted technical assistance to state teams utilizing a learning collaborative model; and 4.Engage integrated care organizations, managed care plans, and other health care entities regarding the needs of consumers and the roles of CBOs. National Partners include: National Council on Independent Living (NCIL); American Network of Community Options and Resources (ANCOR) American Association on Health and Disability (AAHD); National Disability Rights Network (NDRN); ​University of Minnesota; National Council on Aging (NCOA); National Association of State Directors of Developmental Disabilities Services (NASDDDS); Administration for Community Living (ACL).
