Veteran Affairs C2C (COVER to COVER) (IPA Assignment Agreement)
Project Description:
To provide intervention support for Dr. Robert Burke's Research Project "Building a Model VA-State Partnership to Support Non-Institutional Long Term Care for Veterans". The project looks to create a model VA-State partnership in Pennsylvania (PA). This model will leverage and coordinate VA and state investments to promote the ability of high risk Veterans to safely age in place is this is their desire. The COVER to COVER "Connecting Older Veterans (Especially Rural) to Community or Veteran Eligible Resources, developed in Utah, will provide a model to bring PA Veteran Affairs Medical Centers (VAMC) and Managed Care Organizations (MCO) together to address gaps and interventions to improve / enhance high risk Veterans receiving services. Often VAMC and MCO staff do not understand benefits one another offers which often creates major gaps in services. Through benefits training, relationships will be developed to create new systems to address the needs of Veterans.
