Living Well Missouri: A Model for Community Monitoring and Capacity Building

Project Description:
The University of Missouri Kansas City-Institute for Human Development, together with the Missouri Division of Developmental Disabilities, the Missouri Association of County Developmental Services, and other partners, will develop a comprehensive, coordinated system to monitor, track, and enhance the health, safety, well-being, and overall quality of life for individuals with developmental disabilities (DD). The project will build on existing quality assurance processes to develop a standardized data collection, reporting, and analysis system that county DD service boards can use to create innovative capacity-building strategies and activities customized to local needs and the state agency can use to monitor and assure compliance with HCBS rules by all counties. Objectives will be 1) Engage diverse partners and promote meaningful engagement of self-advocates and families; 2) Develop and implement a coordinated system for monitoring and improving safety, health, and well-being of individuals with DD receiving HCBS 3) To design, identify, and implement evidence based practices and innovative strategies, including training to build the competencies of professionals, designed to improve services and reduce risk factors; 4) To disseminate project results, products and relication materials statewide and nationally; 5) To sustain the model system of community monitoring and capacity building. Expected products are a data visualization dashboard, a series of trainings aligned with the Charting the LifeCourse framework to support implementation of the model by individuals, families, professionals, and agencies, and a replication toolkit.
