Exploration of the Nurse's Role in Healthcare Transition Planning for Youths with Chronic Illness and/or Disability

Project Description:
The purpose of the study is to explore the nurses role and responsibilities in health care transition planning (HCTP) for youths with chronic illness and/or disability. The aims of the study were to determine: a) what extent nurses are involved with providing HCTP services; b) activities that nurses engage in when providing transition planning services; and c) the association among selected professional demographics with the provisions of health care transition planning services. A total of 1804 pediatric nurses participated in this survey. Data collection has been completed; data analysis is currently underway. Preliminary findings of this study suggest HCTP training for pediatric nurses at the staff and advanced practice level who provide services to youth with special health care needs.
