Project Description:
Background: Healthcare Transition Initiative is a 5-year plan funded by Oklahoma Department of Human Services. Keeping up with the current practices and evidence, Sooner SUCCESS proposes to define healthcare transition through strategies, objectives and goals under three different domains: State Priority Need Improve the transition to adult health care for children and youth with special health care needs. Oklahoma State Department of Health (OSDH) in collaboration with Department of Human Services (DHS) and Sooner SUCCESS has developed a state action plan, around this priority need. This action plan elaborates on objectives set, along with the strategies that will be employed to achieve both national outcome measures and national performance measures systematized by Maternal and Child Health Bureau.
