Project Description:
The Milagro prenatal Support Program provides the services of a skilled Prenatal Early Intervention Specialist (EIS) to support patients obtaining prenatal care via the Milagro/Family Practice OB clinics on Tuesdays, Wednesdays, Thursdays, and Fridays. This service includes in-the-moment support for women in need of community resources, linkages with other programs, and/or who are interested in engaging in more intensive, relationship-base case management/support services with a FOCUS prenatal Early Intervention Specialist (EIS). The in-vivo EIS will coordinate referrals for clients in need during clinic time and will follow up to ensure linkages are made and allocation of duties between herself and the Milagro Community Support Worker (CSW) are well defined and coordinated. FOCUS provides a dedicated, consistent EIS or other qualified personnel for this purpose, and provides back-up coverage if/when this individual is unavailable due to illness, annual leave or other foreseen absence. The program provides ongoing, relationship-based home visiting services for a subset of pregnant women living in Bernalillo County who are actively receiving prenatal care with the Milagro outpatient program. The prenatal EIS will support patients transition to the FOCUS Part C Early Intervention Program after the birth of the baby.
