ECHO Project - Family Navigator/Outreach
Project Description:
Family Navigation: Expand and improve the Family Navigator training to increase the number of navigators trained, improve the skills of the navigators through additional training opportunities.
A. Explore credentialing options including mental health peer navigator and any other existing or emerging credentials that may lead to long term sustainable funding.
B. Offer the Family Navigation training, which combines the skill sets of navigation and coordination, a minimum of 6 times.
C. Offer the Family Navigation training a minimum of 6 times
D. Develop a web-based portal where navigators can access information and resources and where agencies can request Family Navigator Training.
E. Establish monthly networking meetings of Family Navigators that include training and skill development
F. Utilize CHD zoom archive for training resources
2. Outreach Clinics: Coordinate, manage and attend rural outreach neurodevelopmental clinics
A. Coordinate, manage, and attend at least 7-outreach neurodevelopmental screening clinics.
1. Coordinate marketing of clinics with community agencies.
2. Process referrals; maintain contact with medical providers, care coordinators and the Division of Public Health - Womens, Children's and Family Health; contact families and arrange appointments.
3. Assist clinic team travel arrangements
4. Manage on-site documentation
5. Coordinate LEND fellow activities
6. Review and disseminate post-clinic reports; manage record retention in compliance with HIPPA
7. Collect and analyze client evaluation of clinic services, compile clinic reports.
8. Support community trainings in host communities during the clinic visit period.
3. ECHO Neurodevelopment: Enhance statewide knowledge, screening and management of Autism Spectrum Disorder and related disabilities for primary care using a Project ECHO model. ECHO Family: Increase knowledge and establish best practices focused on youth to adult transitions for individuals who experience developmental disabilities.
A. Coordinate and convene 12 clinics per ECHO project (September 2018 May 2019).
B. Disseminate recommendations
C. Manage HUB contracts
D. Recruit spoke participants and case presenters
E. Manage iECHO and Box
F. Conduct project evaluation by June 30, 2019.
G. Attend MetaECHO Conference in February. This is a second phase training necessary for new ECHOs to grow the infrastructure to support new ECHO growth. This will be a program development and networking opportunity. This new training will increase our capacity for support of current and new spoke members.
